1. Gradauate an accredited physician assistant program
  2. Pass the NCCPA PANCE
  3. Complete the Federation of State Medical Board’s (FSMB) Federation Credentials Verification Service (FCVS).  (Currently applications for PA licensure in the USVI does not require the Uniform Application (UA) from FSMB).
    • This process is estimated to take 4-6 weeks
  4. VI PA Application.  In addition to the completed application, applicants must include:
    • Passport size photo
    • Non-Addicted Affidavit
    • 2 character reference letters
    • Chronological account of all time spent between receiving PA certification/degree and license application
    • Copy of PA diploma/degree
    • Copy of Birth certificate/passport (applicants must be 21 years of age or older)
    • License verification forms completed by each state in which you have held a license
    • Notorized authorized release of information
    • Submit to the US Virgin Islands Board of Medical Licensure (On St. Croix: Currently located at the Office of Licensing and Consumer Affairs by Pueblo, Golden Rock.  On St. Thomas:)
    • Application Fee: $125 to the Government of the VI
    • The Board of Medical Licensure currently meets every 3 months.
  5. Register License with the Board of Medical Licensure
  6. Obtain an NPI number
  7. Obtain DEA number, if desired
